Catchment area

The postcode areas’ that are within Chipping Norton Health Centre’s practice boundary are as follows:

  • OX7
  • OX15 – Wigginton and Hook Norton only
  • GL56 – Adelstrop, Barton on Heath, Chastleton, Daylesford, Kitebrook, Little Compton, Lower Oddington, Upper Oddington, Oddington only
  • CV36 – Ascott, Long Compton and Whichford only

We advise all patients to download the NHS app so you can access test results, order repeat prescriptions and view parts of your medical records all online. Our care navigation team will be able to help you with signing up if required.

If you suffer with a condition such as asthma, COPD, diabetes, hypertension, CHD, cancer or have had a stroke or TIA we ask you to complete the registration forms in full so we can gain a full picture of your current and past health.

Registering with the surgery

To register an adult or child with the surgery:

For any children in the family, we require copies of the immunisations and information listed in your child’s red book. Please complete an online registration form of your own and then supply us with a copy of their birth certificate and their passport if they have one. We will not be able to register your child without seeing their birth certificate and/or red book. When registering a child, please also provide ID for yourself. Please email this information to, cnhc.registrations@nhs.net.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Out of area registrations

We do not accept patient registrations from anyone that lives outside of our practice boundaries as detailed above.

If you are an existing patient (in area) and you move out of area, you will be given a 3 week ‘grace’ period in order to register locally to your new address. After this time has elapsed we will de-register you from the practice.
