Use this service to register a child with the surgery.
Once the form is completed, please email cnhc.registrations@nhs.net with scanned copies of your child’s birth certificate and red book as well as a scanned copy of your ID to complete their registration.
If you do not provide the documents within 6 weeks of completing your online registrations forms, the application will lapse and you will be required to complete the form again.
You can use this service if:
- you want to register a child at the surgery
- the child lives within our catchment area
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number
- details of previous GP surgery
- basic health and medical information
