Patient Triage System

Please use our online form to contact the practice, open core surgery hours ONLY.
Please note, we cannot accept any emails or letters from patients with information or requests for our clinical team, all these requests must be submitted via the form, if you are unable to do this, please call us during opening hours.

New appointment system in September 2025

As already communicated via our social media page, our Website and on notices in the Practice, we are moving to a new appointment system in September. We have so far held two patient open morning but only a handful of patients have attended. There are two to go – Tuesday 19th August 9 – 11.30 and Thursday 28th August 9 – 11.30 so if you have any questions, please come along and speak to us.

The new system is explained in the paragraph at the bottom. To confirm, we are NOT removing telephone access for patients. If you cannot fill in the form online for any reason, please call us on the usual number and a Care Navigator will fill in the form for you.

For any patients who use the reception email address (bobicb-ox.cnhc.reception@nhs.net – or any variations with CNHC.reception) these will remain open to Health Professionals ONLY from September 1st. Following this, all patients must submit enquiries through the online form. Until this date, if you have any questions on how the new appointment system will work please email them to the above address to be answered. Please note that this is a different email address to the prescriptions email, which will not change and remains active (although we do prefer you order your medications via the NHS app 😊)

Thank you for your continued support.

The CNHC Team.

‘Clinician led triage – From patient feedback and GMS (General Medical Services) contractual changes, we have made the decision to implement a new appointment booking system, which will be clinician led. For the majority of patients, this will mean filling in an online form to request an appointment. For those not able to do so, telephone access and other contact methods will still be available. This process is known as Clinician-led triage and is in operation in many GP surgeries across the UK. It means a clinician will be involved in deciding on the next step when a patient request comes into the practice.’
