Appointments And Our Team
At Chipping Norton Health Centre we have built a Multidisciplinary Team which includes allied professionals that you might not usually find within general practice.
As well as our GP, Practice Nurse and Health Care Assistant appointments we can also offer appointments with:
- A Clinical Pharmacist
- First contact physio
- and Advanced Clinical Practitioners (who can treat minor illness and injuries and prescribe medication)
Chipping Norton Opticians are based at the Health Centre and are contracted by the CCG to carry out a Minor eye conditions service for our patients and those registered with other surrounding surgeries.
Care navigation is an emerging concept in the UK, intertwined with that of care coordination – which represents the idea that simply having services and trained people in place are not enough. What then matters is how people (individuals, teams, services and systems) work together – in a ‘joined up’ way– so that people know when and how they can get access the right help, at the right time, in the right place.
Our Care navigation Team (formally known as our reception team) has undertaken specific training led by our allied professionals to enable them to signpost you towards the most suitable clinician from our Multi-disciplinary Team to treat you. In order for them to do this they will need to ask you some questions. Your answers to these questions will enable them to direct you to
The most appropriate appointment, first time.
The benefit of having a Multi-disciplinary Team is access to specialist knowledge and specific and appropriate clinical skill sets. The team communicate and co-ordinate treatment plans where needed and the allied professionals will feed back to your own GP when necessary.
The Care Navigation Team will book an appointment with the clinician that will provide you with the best treatment or care for your condition. If your condition is one that would be best treated by a skilled clinician such as a Physiotherapist, an Optician or a Nurse Practitioner, then your appointment will be booked with the correct specialist.
